Gnolloch means "cursed gnoll". Once in a great while, a gnoll will, through strength of will, win free of its hunger and turn its back on Yeenoghu. When this happens, the gnoll is cursed by Yeenoghu to never hunger again. So a gnolloch will only eat when it remembers to do so. Because of this a gnolloch is often just one step away from exhaustion.
Gnollochs are exiled from gnoll society, and are one of only a few things that a gnoll will not eat (from fear of also being cursed). Gnollochs are often lone wanderers, but sometimes will find a sympathetic ear and be allowed to join a band of other humanoids.

Some gnollochs become adventurer NPCs. For those gnollochs, apply the following adjustments: Then adjust the scores as follows:
- Str +2, Con -2 (to a minimum of 8), Wis +2
- Darkvision 60 feet.
- Add the Clear Minded trait from above.
- Add the Near Starvation trait from above.
- Add proficiency to Survival skill (if not already added).
- Add Gnoll to languages known.
